What is Tresiba 100 Units/ml Penfill ?

TRESIBA PENFILL 100IU contains insulin degludec which belongs to the group of medications called ultralong-acting basal insulin. It is used to treat diabetes mellitus in adults, adolescents and in children aged 1 year and above. Diabetes mellitus is a chronic disease associated with abnormally high levels of glucose in the blood due to inadequate production of a hormone called insulin (produced by pancreas to metabolize glucose) or inadequate sensitivity of cells to the action of insulin.

Insulin degludec is indicated to improve glycemic control in patients with diabetes mellitus. It binds to the alpha subunit of insulin receptor and stimulates the phosphorylation of various insulin receptor substrates proteins, which results in activation of those genes which play critical roles in glucose metabolism.

The most common side effects of taking this medicine are hypoglycemia. Insulin degludec is contraindicated in patients with episodes of hypoglycemia. It is strictly not recommended for the treatment of diabetic ketoacidosis. Inject this medicine subcutaneously into the thigh, upper arm, or abdomen. Patients using TRESIBA PENFILL 100IU should never share needles or syringes with another person.

How TRESIBA PENFILL 100IU works?

Insulin degludec binds to the alpha subunit of insulin receptor and stimulates the tyrosine kinase activity. This stimulation causes phosphorylation of various insulin receptor substrates proteins. This inturn, leads to the activation of downstream signalling molecules including PI3 kinase and Akt, which plays a critical role in glucose metabolism.

Direction for use:

Always take this medicine as directed by your physician. Your doctor will decide the dose and duration of the therapy depending upon your age, weight and condition. Inspect visually for particulate matter and discoloration. Only use this medicine, if the solution appears clear and colorless. Inject this medicine subcutaneously into the thigh, upper arm, or abdomen. Do not administer this medicine or with an insulin infusion pump.

• In adults, inject subcutaneously once-daily at any time of day
• In pediatric patients, inject subcutaneously once-daily at the same time every day

Side effects:

Common:

• Hypoglycemia
• Lipodystrophy
• Edema
• Weight gain

How to manage side effects:

Hypoglycemia:

Try to self-monitor your blood glucose levels. Increase monitoring with changes to insulin dose, co-administered glucose lowering medications, meal pattern and physical activity.

Lipodystrophy:

Do not inject this medicine in a same site. Always rotate injection sites within the same region from one injection to the next to reduce the risk of lipodystrophy.

Weight gain:

Try to exercise regularly. Eat a healthy balanced diet.

Pregnancy:

TRESIBA PENFILL 100IU should be used with caution in pregnant women. Your doctor will decide a suitable dose for you depending upon your condition. Consult your doctor before taking.

Breast-feeding:

TRESIBA PENFILL 100IU should be used with caution in breastfeeding women. Consult your doctor before taking.

Alcohol:

Avoid consumption of alcohol while taking this medicine.

Driving:

Do not drive or use any tools or machines if your ability is affected by this medicine.

Liver disease:

TRESIBA PENFILL 100IU should be taken with caution in patients with severe liver disease. Consult your doctor for advice.

Kidney disease:

TRESIBA PENFILL 100IU should be taken with caution in patients with severe kidney disease. Consult your doctor for advice.

Allergy:

Do not take TRESIBA PENFILL 100IU if you are allergic (hypersensitive) to insulin degludec or any other ingredients in this medicine.

Others:

• This medicine is strictly not recommended for the treatment of diabetic ketoacidosis
• Do not use this medicine if you have episodes of hypoglycemia (decreased blood glucose level)
• This medicine should be used with caution in patients with eye disorders. Consult your doctor for advice

Interactions:

Tell your doctor about all the medicines you use, including prescription and non-prescription medicines, and herbal supplements.

Talk to your doctor, if you are taking,

• other antidiabetic agents (e.g. pioglitazone, sitagliptin, saxagliptin, linagliptin, liraglutide, canagliflozin, dapagliflozin or empagliflozin)
• medicines used to treat high blood pressure like thiazide diuretics, beta-blockers (atenolol, propranolol), ACE inhibitors (captopril, enalapril, lisinopril) or angiotensin II receptor blocking agents (losartan, valsartan)
• disopyramide (a medicine used to treat arrhythmia)
• medicines used to treat depression (e.g. fluoxetine, lithium)
• fibrates (used to reduce cholesterol)
• sulfonamides antibiotics (used to treat infections)
• aspirin (and other salicylates), for pain and mild fever
• octreotide and lanreotide (used to treat acromegaly)
• medicines used to treat psychotic disorders e.g. atypical antipsychotics (olanzapine and clozapine)
• danazol (used to treat endometriosis)
• oral contraceptives (birth control pills)
• thyroid hormones, for thyroid problems
• growth hormone, for growth hormone deficiency
• corticosteroids for inflammation
• sympathomimetics such as epinephrine (adrenaline), salbutamol or terbutaline for asthma
